GOAL:
Reduce diabetes health disparities by linking to wellness and healthy lifestyle behavior programs, building capacity with training, increase awareness through education, and establishing a Minority Health Coalition.
Objective 1:
Provide local healthy lifestyle programs and preventive health screenings.
Objective 2:
Increase awareness of the impact of mental health on diabetes.
Objective 3:
Increase the capacity of minority representation to address health disparities.
This program will focus on strengthening neighborhoods situated in preselect zip codes using community member engagement. We know that people with a greater sense of community are more likely to act in healthy ways and work with others to promote well-being for all.
For this reason, the scope of the project will be implementing a multidimensional program that engages existing and newly formed initiatives to synergistically transform strategic areas within Calvert to improve health equity, build the capacity of minority Community Health Workers’ representation within their neighborhoods as well as build a continuum of care to provide continuity of care and appropriate navigation.
These programs bring hands-on nutrition education, health screenings, and healthy lifestyle promotion to neighborhoods. These culturally appropriate peer-led lifestyle interventions will promote and encourage healthier lifestyle changes amongst the participants.
The program works with various local organizations in communities and provides them with support, training, and equipment so people can make healthier choices. The program helps educate entire communities about the importance of healthy habits and it costs less than treating the problem at an individual level.
