Health equity is achieved when every person has the opportunity to “attain his or her full health potential” and no one is “disadvantaged from achieving this potential because of social position or other socially determined circumstances.” Health inequities are reflected in differences in length of life; quality of life; rates of disease, disability, and death; severity of disease; and access to treatment.
Mission:
To address the social determinants of health and eliminate health disparities by harnessing the power of collaborative relationship with community partners working synergistically together to build bridges to eliminate the gaps in cares, empowering communities to transform by building capacity of minority community health workers within the communities of color as well as influencing strategic direction to improve health outcome.

According to the CDC social determinants of health are factors in the social environment that contribute to or detract from the health of individuals and communities. Social determinants of health have been found to directly increase the burden of heart disease and stroke and their risk factors. They also indirectly influence health-promoting behaviors.
